She can take over your lease if the two of you and your landlord agree to it, but she can't change the locks and refuse to give you your things (legally). Why don't you call the police and have them make her open the door? Unless there is more to this...you haven't been harassing her or threatening her have you? ...and she doesn't have a restraining order against you?

File a law suit against her and ask for your things back or for the value of them.
You can call the police, and when an officer shows up, you can explain what's going on and ask for assistance retrieving your belongings. She does not have the right to keep them.
